Hill Care Health provides family practice and psychiatric care services, including but not limited to:

All services are provided within the scope of our practitioners’ qualifications and licensure. When your needs exceed our capabilities, we will provide appropriate referrals.
3. Confidentiality & HIPAA Compliance
All personal and medical information shared during the course of your care is confidential and handled in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Information may be disclosed in limited circumstances, including:

6. Appointment & Cancellation Policy
We ask that you provide at least 24–48 hours’ notice if you need to cancel or reschedule an appointment. Late cancellations and no-shows may result in:
- A cancellation fee charged directly to you (not your insurance)
- Reduced scheduling priority for future appointments
We understand that emergencies happen. If you have an unavoidable conflict, please contact us as soon as possible.
7. Financial Terms & Payment
- Fees for services are itemized and provided to you before treatment when possible
- Payment is due at the time of service unless prior arrangements have been made
- We accept various payment methods including major credit/debit cards and, where applicable, insurance
- You are responsible for understanding your insurance coverage, including copays, deductibles, and out-of-network limitations
- Unpaid balances may be subject to collection procedures after reasonable notice
8. Client Responsibilities
To support effective care, you agree to:
- Provide accurate and complete information about your medical history, current medications, and symptoms
- Actively participate in your treatment plan
- Inform your provider of any changes in your condition or medication
- Treat our staff and other patients with respect
- Comply with practice policies, including those regarding payment and appointments
9. Wellness Partners & Independent Providers
Certain wellness practitioners operate independently within our facility. These providers lease space from Hill Care Health but are not our employees or agents. As such:
- Hill Care Health does not oversee, manage, or assume responsibility for their services
- All appointments, payments, and services with independent wellness providers are handled directly between you and the provider
- Each independent provider maintains their own terms of service, privacy policies, and professional liability coverage
10. Crisis & Emergency Services
Hill Care Health is not an emergency service provider. If you are experiencing a medical emergency or mental health crisis:
- Call 911 immediately
- Go to your nearest emergency room
- Contact the 988 Suicide & Crisis Lifeline by calling or texting 988
After-hours assistance from our practice is provided at the discretion of the treating provider and is not guaranteed.
11. Termination of Services
Either party may end the therapeutic or clinical relationship. Hill Care Health may terminate services in the following circumstances:
- Your treatment needs exceed our scope of expertise or licensure
- Non-compliance with treatment plans or practice policies
- Non-payment for services rendered
- Behavior that threatens the safety of staff or other patients
- Violation of professional boundaries
When possible, we will provide reasonable notice, assist with transition of care, and offer referrals to other qualified providers.
12. Intellectual Property
All content on this Site — including text, graphics, logos, images, and design elements — is the property of Hill Care Health or its licensors and is protected by copyright and trademark laws. You may not:
